I ________________________________________________ hereby give my consent to &5678 Dance Studio to take or use photographs, digital images, and video/audio recordings of me for use in news releases and/or printed publications, materials, posters, brochures, greeting cards, electronic publications, social media sites (Facebook, Instagram) or Web sites.

I agree that &5678 Dance Studio may use, edit or reproduce such photographs and videos for purposes related only to &5678 Dance Studio.
 

I authorize the use of these images without compensation to me. All negatives, prints, and digital reproductions shall be the property of &5678 Dance Studio.

I hereby waive, release and forever discharge &5678 Dance Studio from any and all claims or liability arising from the use of my photograph for the above purposes. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization.

I represent that I have the full legal capacity and right to execute these releases and grant the rights granted herein.
